Services
Outpatient Mental Health Clinic provides readily
accessible services in Philadelphia to meet the special mental health and
emotional needs of dependent children and their families, as well as the special
needs of adoptive families. Tabor's psychiatric consultant monitors all
treatment plans, with a range of treatment strategies employed for younger
children, school-age children, adolescents and families, including post-legal
adoptive families.
Community Living Arrangement is a supported
independent living home in Philadelphia for young adult males with developmental
disabilities. Staff members provide 24-hour-a-day supervision, support the
efforts of these young men to achieve their personal independence goals and
encourage their maximum self-sufficiency.
Emerging Adults Transition Program provides housing and supportive services for young women who are able to manage their behavioral health diagnoses independently. Designed for single women aged 18 - 21 years and young women with only one child under age 5, the program assists youth with behavioral needs in preparing to transition from the children’s system to adult systems. Program supports and services build on the women’s strengths and promote their success in becoming more independent. Skilled case managers and supervisors engage the women in one-to-one organized activities that include learning how to locate and obtain housing, negotiate a housing lease, acquire goods and services, and understand common daily living activities. Tabor staff assist each woman in creating a plan that identifies educational, vocational, employment and social/personal goals to stabilize her life and prepare to become a contributing member of the community. Duration of services can vary based on the progress and need of each individual, but may last up to three years.
